 The Mundelein Fire Department is proud to announce the “File for Life” program. What this program does is “Help Us Help You”, by making your medical information available to the responding Firefighter/Paramedics in a medical emergency at your home.
Click here to open the “File for Life” form and you will be able to fill it out right on your computer. After your done print one for yourself and place it on your refrigerator. Then click the email button at the bottom of the form and your address will be sent to the Mundelein Fire Department. A symbol will be placed on your address on a mapping program that the Firefighters use while responding to emergencies. This will give them advance notice that the information will be available for them when they arrive at your home.
You can also go to one of your fire stations and pick up a kit that contains the following. The medical information forms, a clear page protector with a sticker, a door sticker, and an adhesive strips to allow you to attach it to your refrigerator. These will also soon be available at your local pharmacies in town. If you need assistance filling it out you can contact us and we will arrange for a Firefighter to come out and assist you. Your updated medical information will help us and the hospital make sure that you get the best care possible. So “Help Us to Help You” fill out a “File for Life” form today!
